I, the undersigned client, hereby give my consent to receive a tattoo/piercing at District 36 Tattoo Parlor under the following terms and conditions:

1. I acknowledge that I am of legal age to provide consent for the tattoo/piercing procedure and that I am voluntarily choosing to receive the tattoo/piercing.

2. I am not under the influence of drugs, alcohol, or any other substances that may impair my judgement.

3. I am not pregnant or breastfeeding.

4. I have been informed about the design, location, and size (if applicable) of the tattoo/piercing, as described above, and I consent to receiving the specified tattoo/piercing.

5. I understand that the tattoo/piercing process involves potential risks, including but not limited to infection, bleeding, scarring, allergic reactions, and discomfort. I accept these risks.

6. I am aware of the aftercare instructions provided by District 36 Tattoo Parlor and agree to follow them to ensure proper healing and minimize complications.

7. I understand that District 36 Tattoo Parlor will take necessary precautions to maintain a safe and sterile environment during the procedure.

8. I acknowledge that I have had the opportunity to discuss the tattoo/piercing procedure with the artist/piercer and have received satisfactory answers to any questions I may have had.

9. I hereby release and hold harmless District 36 Tattoo Parlor, its artists/piercers, and employees from any liability or claims arising from the tattoo/piercing procedure or its aftercare.

10. I understand that the final result of the tattoo/piercing may vary due to individual factors, and District 36 Tattoo Parlor is not responsible for any dissatisfaction with the result.

11. I acknowledge that this consent form will be kept on file by District 36 Tattoo Parlor and will be made available for inspection upon request by the appropriate authorities.
