I, the undersigned parent/guardian, hereby give my consent for my minor child, named above, to receive a tattoo/piercing at District 36 Tattoo Parlor under the following terms and conditions:

1. I acknowledge that I am the parent/legal guardian of the minor named above, and I have the legal authority to grant consent for the tattoo/piercing procedure.

2. I understand that tattooing is a permanent procedure and that the minor is voluntarily choosing to receive a tattoo.

3. I am aware of the design and location of the tattoo, as described above, and I consent to the minor receiving the specified tattoo.

4. I have been informed about the potential risks and complications associated with the tattooing/piercing process, including but not limited to infection, scarring, and allergic reactions, and I accept these risks.

5. I agree to ensure that my minor child follows all aftercare instructions provided by the tattoo artist/piercer to promote proper healing and minimize complications.

6. I understand that District 36 Tattoo Parlor will take all necessary precautions to maintain a safe and sterile environment during the tattoo/piercing procedure.

7. I acknowledge that I have had the opportunity to discuss the tattoo/piercing procedure with the artist and have received satisfactory answers to any questions I may have had.

8. I hereby release and hold harmless District 36 Tattoo Parlor, its artists, and employees from any liability or claims arising from the tattoo/piercing procedure or its aftercare.

9. I agree to be present at the tattoo shop during the entire tattooing/piercing process and to provide my identification and documentation, as required by law, to prove my legal relationship to the minor.

10. I understand that this consent form will be kept on file by District 36 Tattoo Parlor and will be made available for inspection upon request by the appropriate authorities.
